Real-world evidence used to mean a retrospective chart audit at the end of a product's life. That view is a decade out of date. Modern RWE is a continuously governed programme: prospectively designed, methodologically declared, and running in parallel with, or in place of, traditional data collection where the question allows. The change is not just technological. It is philosophical: treating real-world data as evidence-in-waiting only if it was collected with the discipline you would apply to a trial. Provenance, metadata, missingness handling and comparability all belong at the front of the study, not at the reviewer's desk. Sponsors who make this shift stop apologising for their RWE and start defending it.
